Location: 275 Prospect Street, Norwood
Status: Regular Full-Time Position
Hours: 40 hours/week, Monday- Friday, 8:00 a.m. – 4:30 p.m.
Salary: FY22 Grade N6, salary range weekly $1,033.17-$1,267.68

General Summary: The purpose of this position is to perform social service and administrative work in maintaining
contact with the Town’s senior citizens to assess their needs, provide a wide variety of information, and make referrals as
necessary. Works as a member of the COA team and reports to the Director of the Council on Aging.

Position duties:
▪ Assesses client’s situation and needs. Provides accurate and current information regarding the resources available
including housing, health care, home health care, homemakers, transportation, fuel assistance, nutrition, and a
variety of legal and financial matters. Provides guidance and assistance in the preparation of forms and necessary
documents.
▪ Provides client visitation to homes and hospitals when needed. Visits elderly housing complexes for information,
referral, problem solving, and public relations.
▪ Attends outreach meetings and informational sessions with human services agencies for exchange of policies on
senior issues and concerns.
▪ Accompanies police or health officials on visits to homes to resolve immediate emergency situations.
▪ Prepares and presents necessary outreach reports to the Council on Aging and the Director when required.
▪ Transports medical supplies such as wheelchairs, raised toilets, walkers, and bed rails.
▪ Performs similar or related work as required.

Requirements:
▪ Bachelor’s degree in social work, human services, gerontology, or related field; three to five years of
progressively responsible professional experience in geriatrics and human services, or equivalent combination of
education and experience. Previous outreach worker experience a plus.
▪ Possession of a valid motor vehicle operator’s license.

Special Requirements:
▪ Licensed Social Worker status preferred but not required.
▪ Certification in First Aid, CPR and defibrillator preferred but not required at time of hire.
▪ Positive completion of a CORI test and any other reasonable background check.
▪ Ability to interact with the elderly in an empathetic, compassionate, and sensitive manner.
▪ Ability to maintain confidential information.
▪ Must demonstrate initiative and creativity in carrying out daily responsibilities.
